QUALIFICATIONS:

REQUIREMENTS:

Municipality of San Juan is composed of 42 barangays.  It has an estimated total population of 114,068 as of 2020 PSA census.

Blood is essential to one’s survival, acting as a way for oxygen and essential nutrients to reach different organs in the body.

It has estimates that more than 2,000 blood units are being transfused to patients all over the country everyday. You also cannot discount the possibility that someone you know might need blood transfusion in the near future.

But, patients aren’t the only ones benefiting from blood donations. Donating blood regularly also presents a multitude of health benefits for the donor.

Republic Act No. 7719 otherwise known as National Blood Services Act of 1994 aims to create public consciousness on the importance of voluntary, non-remunerated blood donation (VNRBD) in saving the lives of millions of Filipinos.

OBJECTIVES:


The primary objective of the said activity is to collect a variety of blood products that will be utilized in saving the lives of patients especially among regular dialysis patients of this Municipality who are in need of blood. 

 

The following are benefits for blood donor:

1. Free health checkup and pathology test

2. Reduce the risk of heart and liver diseases

3. Lower the risk of cancer

4. Develop new blood cells

5. Maintain your body weight

6. Lower cholesterol level

7. Improve your mental wellbeing

8. Save lives

 

Donating blood can be the highest form of humanitarian act. Without blood donation, patients can’t undergo surgeries, recover from illnesses, or just function on a daily basis. You may not be able to help cure someone from life-threatening diseases like cancer, but you can definitely help someone stay alive when you donate blood.

GUIDELINES FOR THE IMPLEMENTATION OF EDUCATIONAL ASSISTANCE PROGRAM OF THE 

MUNICIPALITY OF SAN JUAN, BATANGAS 

In accordance with the mandate of the Commission on Higher Education (CHED) as provided for in Article XIV, Section I of the Philippines Constitution “To protect and promote the right of all citizens to quality education at all levels and shall take appropriate steps to make such education accessible to all” and Article XIV Section 2(3) “To establish and maintain a system of scholarship, grants, student loan program, subsidies ang other incentives which shall be available to deserving students in both public and private schools”. The Local Chief Executive together with the Committee Chairman on Education of the Sangguniang Bayan and the Scholar Head, formulated and approved the Guidelines for the Implementation of EDUCATIONAL ASSISTANCE PROGRAM for A.Y. 2022-2023.

I. OBJECTIVES


The general objective of the guidelines is to rationalize the administration and implementation of the Educational Assistance. Specifically aims to:

 

I.1   Provide guidelines in the selection of deserving grantees;

I.2   Ensure that the grantees are enrolled in duly authorized educational institutions; and

I.3   Ascertain that the Educational Assistance are properly administered, distributed and implemented.


II. COVERAGE

 

Educational Assistance shall cover the following programs:

 

a. Assistance for University/College students – intended for degree program enrollees in public and private institutions.

b. Assistance for Senior High School students – intended for enrollees in Senior high school in public and private institutions.

 

III. QUALIFICATION REQUIREMENTS

Grantees must be:

A. A bona fide resident of San Juan, Batangas

B. Currently enrolled in any public or private school – Senior High School and College subject to additional qualifications;

C. Strictly comply with all the documentary and qualification requirements of the LGU.

 

APPLICATION PROCEDURE AND DOCUMENTARY REQUIREMENTS FOR SCHOLARS

Documentary Requirements

1.Original Certificate of Indigency from the Barangay

-with Dry Seal

-with Original signature of Brgy. Captain 

2. Registration Form / Certificate of Enrollment / Assessment Form

-Photocopy / Printed

-With complete signatories 

3. Copy of Valid School ID (AY 2022-2023) with three (3) original signatures. 

4. Duly-accomplished APPLICATION FORM

PROCEDURES

1. The applicant/parent shall report and submit the documentary requirements (COMPLETE) to their authorized personnel on his/her respective school for verification.

2. Authorized personnel of each respective school shall submit the consolidated applications with documentary requirements to the Office of the Mayor for further evaluation.

3. After verification and evaluation, the requirements shall be forwarded to responsible departments for further checking and documentation.

4. RELEASE.


MODES OF PAYMENT

The LGU will release the financial assistance directly to the student only.

Job Order

1. Provide information, education and communication;

2. Orients the responsible parties who will be manning the distribution, the policies, standards and guidelines;

3. Determine the allocation and corresponding fund requirements;

4. Gather and consolidate reports on scholarship program’s data, utilization of slots and fund allocation.

AMOUNT OF EDUCATIONAL ASSISTANCE - PHP 2,000.00


SEPARABILITY CLAUSE

If any part of this order is declared void or unconstitutional by operation of law, rules and regulations of the same shall apply only to that specific provision and the remaining clause shall remain valid and enforceable.


REPEALING CLAUSE

All other existing orders and memorandum issued by the Office of the Mayor inconsistent to any provisions of this guideline will be deemed modified.


EFFECTIVITY

The Administrative Order shall take effect immediately upon approval.

Approved on the 21st day of March, 2023 at the Office of the Municipal Mayor.

DISTRIBUTION OF EDUCATIONAL CASH ASSISTANCE

Maluwalhating naipamahagi ang Educational Cash Assistance na nagkakahalaga ng Php 2,000.00 bawat kwalipikadong mag-aaral sa lahat ng Public Senior High School sa ating bayan noong July 6-7, 2023 ganap na ika-8 ng umaga hanggang ika-3 ng hapon sa Evacuation Center, Brgy. Buhaynasapa, San Juan, Batangas. 

Ito ay sa pangunguna ng ating Punong Bayan, Kgg. Ildebrando D. Salud, Pangalawang Punong Bayan, Kgg. Octavio Antonio L. Marasigan, SB Chairman ng Committee on Education, Kgg. Angelo LUis T. Marasigan at mga Miyembro ng Sangguniang Bayan.

Ang programa ng ating Lokal na Pamahalaan lalo't higit ang mga programang pang-edukasyon ay maayos na maiipagkaloob sa patuloy na pagsuporta at pakikipagtulungan ng lahat.

CERTIFICATE OF UNEMPLOYMENT

This Certification is issued upon request to be used for ESC Program.

Educational Service Contracting or ESC is a system of government financial assistance that provides grants for deserving elementary graduates to pursue secondary education in a private high school of their choice.

REQUIREMENT:

Parent / Guardian must submit an original copy of Certificate of Indigency from respective Barangay.

ENDORSEMENT LETTER FOR ELECTRICAL PERMIT

ENDORSEMENT LETTER is issued upon request to be used for ELECTRICAL PERMIT FEE.

REQUIREMENT:

Submit an original copy of Certificate of Indigency from respective Barangay.
